Client Responsibilities

Service Accuracy and Liability

  • AZ Balance LLC is not responsible for delays or errors resulting from incorrect information provided by the client.
  • We are not liable for claim denials that occur due to factors outside our control, such as payer policies or patient ineligibility.
  • Our role is to facilitate billing and claims submission, and we do not guarantee reimbursement from insurance providers or third parties.

Limitation of Liability: AZ Balance LLC’s liability is limited to the fees paid by the client for the services rendered. We are not liable for any indirect, consequential, or punitive damages arising from the use of our services.

Termination of Services

Either party may terminate the service agreement under the following conditions:

Client-Initiated Termination:

  • Clients may terminate the agreement with written notice of 30 days
  • In case of cancellation after services have been initiated, the client is responsible for paying for the services rendered up to the termination date.
  • No refunds will be provided for completed services.

AZ Balance LLC-Initiated Termination:

  • AZ Balance LLC reserves the right to terminate the agreement for non-payment, breach of contract, or failure to provide necessary information for billing.
  • Upon termination, any outstanding balances will be due immediately. A pro-rata refund may be issued for unrendered services, minus administrative costs.

Dispute Resolution

In the event of a dispute, AZ Balance LLC is committed to resolving issues amicably. Should any conflicts arise regarding payments, services, or the interpretation of the service agreement, the following steps apply:

Initial Resolution: Clients should contact our support team with a written description of the issue. We will review and respond within 7-10 business days.

Mediation or Arbitration: If the issue cannot be resolved internally, either party may seek mediation or arbitration in accordance with the terms of the service agreement. Legal action will only be pursued as a last resort.
